Learning & Training

We are keen to support the learning needs of the primary care community – from clinicians to healthcare planners to communities! We have lots of learning resources and offer training in our Team Mapping and PACC Mapping methods on a regular basis.

Training

We offer regular virtual training in both Team Mapping (TM) and Primary and Community Care (PACC) Mapping to help clinical teams, Primary Care Networks, and communities work better together. To be notified about upcoming training sessions for any of these mapping supports please join our Training Registry.

Once trained, you will have access to our facilitator resources and ongoing support through our Community of Practice.

Team UP! Podcast

The Team UP! podcast explores different ideas related to team-based primary care in British Columbia. Team UP! uses the soundsnap library.

Our most recent season, Season 7: A Nurse is Joining Your Practice!, was released in Fall 2024 to support the new “Nurse in Practice” funding program for primary care. In this season we explored nurses as members of primary care teams and covered a range of topics from how to work together to how to promote a nurse to patients, to nursing scope.

Previous Seasons

Season 1: How primary care teams work and how they can work together better.

Season 2: Primary care system resilience and how teams are a protective factor to reduce provider burnout and increase job satisfaction.

Season 3: How to work together in distributed primary care teams.

Season 4: Technology for primary care teams including teamBITS on EMRs, Telehealth, and patient portals.

Season 5: Exploring the wide range of roles in primary care teams. We start with patients and build the team around the patient over this season.

Season 6: Walking through the Learning Pathway to Team-Based Care with a focus on the steps teams go through to get to team based care.

Team Up! Webinars

From 2021 to 2024 we co-facilitated the Team Up! webinar series, collaborating with Health Quality BC, as part of teambasedcarebc.ca. The webinars focussed on topics to support team-based care and often feature clinical teams and their experience with TBC. Dig into the full library of past recordings for some great TBC learning!

Other Resources

Sometimes the best way to learn is to get your hands into some of the tools and play around! Our team has developed some great Tools for teams.

  • TBC Communication-to-Patient Checklist. Improve the ways you communicate with patients when you add new team members or change how the team works together.
  • Nurse Compass. Learn more about the Nurse scopes of practice to decide what type of nurse is the best fit.
  • CapEs. Explore how a range of allied health professionals might change the capacity of your team by considering the overlapping scopes of 10 healthcare professions and how care be shared to impact patient attachment, access to preventative care, chronic care or urgent care. If you want to try out CapEs with your clinic or community, we would be happy to facilitate that! CapEs isn’t yet available for self-directed learning but we have used it lots with PCNs, clinics, and communities to review service plans or changes to the numbers and types of providers in their teams.
