We offer a range of mapping tools that help clinical teams, communities or groups like Primary Care Networks, Divisions of Family Practice and Health Authorities work better together. Each method takes a person-centred approach using simulated cases (personas) to help communication, collaboration, and connection – which improves patient care!
We train healthcare leaders, coaches, and system partners from across BC to use these supports with the teams and communities where they work. What we hear from teams is that the ISU’s mapping tools are a practical, easy and flexible way to bring people together and result in stronger teams and creative solutions to gaps in community healthcare.

Team Mapping
For clinical teams who work together, focussed on individual roles, responsibilities, and gaps with respect to patient care. Team Mapping improves role clarity and co-ordination of care across a team.

Primary and Community Care (PACC) Mapping
For communities, to bring together a wide range of stakeholders to explore a challenge or gap in care and generate solutions.
Governance Workshops (in development)
- For groups like community-based PCN Steering Committees or Joint Leadership Committees, interested in considering how to work together to make decisions, communicate effectively, and design processes to support collaborative work.
Support Services Mapping (in development)
- For those in support roles, who are helping primary care clinics, often through a PCN or Division of Family Practice, including those who provide coaching, quality improvement, recruitment & retention services. Support Service Mapping uses simulated clinic personas to help groups collaboratively map out the wrap around support services available to maximize reach and impact while minimizing duplication.
How can we help?
Training
We offer training in both Team Mapping and PACC Mapping so individuals can use these methods in their work! If you would like to join an upcoming training session for any of these mapping supports please join our Training Registry.
Ongoing practice
We support an active and informative Community of Practice (CoP) for anyone trained in these mapping methods. The CoP meets every 4-6 weeks and provides opportunities for further learning, practicing methods, adapting supports to new contexts, and connecting with others who are using mapping in their work.
Co-facilitation
We can support you as you facilitate your first workshops, through co-facilitation. Our goal is to have trained facilitators.
Advice & Adaptation
We can help to adapt a method to suit a particular audience or achieve a specific outcome, that might be slightly different than what is typical. We can help you choose personas or develop new personas if you need something different.
Workshop Facilitation
We can facilitate any of these workshops with your team or community (mostly virtually, sometimes in-person). If you are located in BC and working in primary care, we can often do this for free! If you are located outside of BC or primary care, we would be happy to pull together a quote for this work.
