Primary and Community Care (PACC) Mapping

We developed PACC Mapping to help communities explore how they could address local primary care needs. We bring together health care providers and planners, community services, government, and community members in a facilitated workshop to think about a need or gap in the community, using patient personas to guide the conversation. We’ve used PACC Mapping to support communities in exploring the following service areas:

  • Adult Mental Health and Substance Use (MHSU)
  • Medically Complex Adults
  • Child & Youth Mental Health and Substance Abuse (MHSU)
  • Maternity Care
  • Frail Elderly Care
  • COVID-19 Immunization Planning and Uptake (see immPACC Mapping).

PACC Mapping can work in all contexts – from rural to urban, large to small, central to remote, and beyond, and our team works with communities to make sure that each workshop is designed to consider the unique needs of your community. Our delivery methods most recently have included both in-person and virtual formats, depending on the values, resources, and priorities of each community.

We would love to help! Do you want to run PACC mapping yourself? We offer training in PACC Mapping a few times a year and invite anyone interested in being trained to join our Training Registry.

If you are hoping for a trained facilitator to work with you to offer PACC mapping in your community, we can do that too! We can support planning, preparation and facilitation of PACC Mapping alongside communities within BC.
